I am 2nd year PEM fellow, I plan to do a global health rotation in Pakistan for two weeks. Pakistan is a hub an endemic disease and also a wide variety of trauma cases due to no regulated safety rules. My goal through that rotation is to gain the experience of managing wide variety of illness in resource limited settings and may be apply that knowledge if needed here in cases of mass casualties and disasters. Also I plan to learn all the procedures there which we perform less often and since we perform those procedures infrequently there are slim chances of being successful thus patient care gets affected.
My goal is provide my pediatric knowledge to the people of larkana , sindh Pakistan. This people belongs to a very region of Pakistan. People are very vulnerable there , no access to clean water, energy thus they are at high to suffer from illness, malnutrition and other problems.
Impact will be a lot of learning experience and education. The knowledge gained can be shared with medical students and residents and ofcourse the skills gained will provide better care for the patients

I chose a very small town for my global health, it was small city of state sindh named larkana. The people and I children I saw belong to extremely poor families. The whole experience was eye opening for me. The amount of infant mortality was sky rocketing. People there miss even basic needs, I saw cases of so much vaccine preventable diseases but either due to lake or education and awareness or ineffectiveness of vaccines kids were dying from that disease. I learnt who to be humble and listen to my patients during this trip. I think I have improved my clinical skills and also improved way to deliver bad news. I did 5-6 times CPR every single day during my rotation. The people in larkana and the government hospitals needs basic tools to provide effective care like laryngoscope, I gel, LMA, Ambu bags, alcohol swabs, gloves and many more things. Also, the maternal ward needs medicines and beds to carry out normal deliveries. Also, I think there should be campaign about how to make infant formula, people were providing extremely diluted formulas to kids and thus kids were dying from malnutrition and electrolyte imbalances, the amount of malnutrition I saw was really surprising. People need education about to prevent hypothermia in babies, I saw so mag neonatal deaths due to hypothermia. I saw deaths due to lake of prenatal care. Overall, the trip was excellent resource to see how other countries practice and the variety of illnesses there. Thank you, Dox foundation, for providing this awesome opportunity of learning, I would love to be considered again for a global health rotation in future again.
